CommonSpirit Health at Home is seeking a travel Physical Therapist for a travel job in Fort Wayne, Indiana.

Travel**Minimum of two years of recent experience as a Physical Therapist in a home health **Minimum of one year using HCHB/Pointcare EMR
PT: Home Health
Start Date: ASAP
Shift: 8-5

Qualifications
- Must have 2 years of recent Home Health Physical Therapy experience in the last 12 months.
- Must have 1 year of recent Homecare Homebase (Point Care) experience in the last 12 months.
- Current American Heart Association BLS

Weekly amount stated in the job postings is based on the estimated salary/ hourly rate upon hire and the stipends for the location of the assignment. Your taxable Salary/hourly rate will be negotiated upon hire and NEVER gets reduced. You can rely on a consistent paycheck without fear of “rate reductions.”
